Once you have scheduled your appointment, you can either fill out the “New Patient Forms” on this website or you can do so when arriving for your appointment.
We ask that all new patients arrive 15 minutes ahead of their scheduled appointment in order to have paperwork filled out by the time they enter the exam room.
- Driver License
- Insurance Card
- Any disc with recent MRI, CT or X-Ray studies
- Any information about your medical history that you find is pertinent to your appointment
Patient Information
Our statements are sent out on a monthly basis through our business office. All charges are due and payable within 30 days of receipt.
Please contact us, if payment in full would be a financial hardship and we can set up a payment arrangement with you.
For billing inquiries, office hours are 8:30a.m. to 5:00 p.m., Monday through Thursday and 8:30 a.m. to 3:00 p.m. on Fridays.
