Once you have scheduled your appointment, you can either fill out the “New Patient Forms” on this website or you can do so when arriving for your appointment.

We ask that all new patients arrive 15 minutes ahead of their scheduled appointment in order to have paperwork filled out by the time they enter the exam room.

2Appointment Policies
We understand that your time is very important and therefore make every effort to see our patients promptly. However, sometimes emergencies arise in the office or at the hospital and our physicians must respond immediately. In the event that this should happen, we appreciate your patience and understanding. If you are unable to wait, we will be happy to reschedule your appointment as quickly as possible.
3What to Bring
Please bring the following items to your appointment:
  • Driver License
  • Insurance Card
  • Any CDs with recently done MRI, CT or X-Ray studies, including reports
  • Any information about your medical history that you find is pertinent to your appointment
4Cancellation Policies

Our goal is to provide quality medical care in a timely manner. In order to do so we had to implement an appointment cancellation policy. This policy enables us to better utilize available appointments for our patients in need of medical care.

Please be courteous and call promptly if you are unable to attend an appointment. This way, we can reallocate the time slot to another patient in need of treatment.

If it is necessary to cancel your scheduled appointment, we require that you call by 10 a.m., one (1) working day in advance. Appointments are in high demand, and your early cancellation will give another person the possibility to have access to timely medical care.

1Billing

Our statements are sent out on a monthly basis through our business office. All charges are due and payable within 30 days of receipt.

Please contact us, if payment in full would be a financial hardship and we can set up a payment arrangement with you.

For billing inquiries, office hours are 9:00 a.m. to 5:00 p.m., Monday through Thursday and 9:00 a.m. to 5:00 p.m. on Fridays.

2Credit Policy

We share our patients’ concern over the increasing costs of medical care and thus we strive to keep our fees reasonable and customary. If you have any questions about your bill with NeuroCare Long Island, please ask us. You are responsible for payment of your account even if you have insurance; your insurance contract is an agreement between you and your insurance company.

Depending on your contract with your insurance company, you may be required to make a co-payment or pay a deductible amount. Co-payments are required on the date of service. Insurance payments paid directly to the provider will be applied to your account. If there is a balance remaining you will receive a billing statement.

3Referrals
If your insurance plan requires a referral for you to see a physician at NeuroCare Long Island, this must be obtained from your primary care physician. Patients are responsible for making sure a referral is in place before services are received. Please call our office if you need further assistance in this regard.
